How to Make a Complaint

 
 

As we are a registered NDIS provider, you have the right to make complaints about our services and supports at any time. We welcome your feedback to help us to continue improving our services and encourage you to contact us. To make a complaint, you can:

  1. Talk to the staff member who provided the service. If you are not happy with what the staff member has said, or you don't want to talk to the staff member, then please contact us at complaints@connextions.org.au or complete the complaint form below.
  2. After we receive your complaint, we will take the necessary steps to investigate. We will keep you updated on all progress.
  3. If we cannot resolve an issue for a client receiving a disability service, you have the right to contact the NDIS Quality and Safeguards Commission on 1800 035 544.
  4. If you are not happy with the NDIS actions, contact the Commonwealth Ombudsman on 1300 362 072.

At any stage of this process, you may appoint an advocate to speak on your behalf. This may be a friend, relative or professional advocacy worker.
