What does a Support Coordinator do?
Support Coordinators play a crucial role in helping you make the most of your NDIS plan. We can assist you with understanding your plan, connecting with service providers and manage your supports.

Your Support Coordinator will meet with you regularly to monitor how your goals are being achieved and assist you to make any changes or adjustments you need. ConneXtions Support Coordinators will keep records about your progress so you have good information to use when you review your plan with the NDIS.
Register with ConneXtions
Your Support Coordinator will ensure that plan reviews commence three months before the end of your current plan and provide updates to the NDIS, monitor change in circumstances, complete progress and support you to fully engage in the National Disability Insurance Scheme to achieve the goals and outcomes identified in your plan.
