    If you are successful, you will be engaged in a work-based learning model where you will be gaining valuable hands-on experience while receiving mentorship from industry experts and the opportunity to work with ConneXtions on a permanent full-time basis.


    Current qualifications are welcomed however not necessary as these can be gained as part of the role, with study costs covered by ConneXtions while you remained employed by us. 


    • We assist people to think about their life goals, plan the supports they need and explore what options exist and/or be created, to best fit the person and their lifestyle.
    • We support people to self-direct and/or self-manage and can customise, coordinate and/or manage services, support plans and team arrangements.
    • We encourage people to do as much or as little as they are comfortable with and provide only the support necessary for people to do this.
    • We can provide assistance with planning support, plan implementation, plan management and/or support coordination.

  • Disability Support Worker - various locations


    We are seeking to engage appropriately skilled and qualified individuals to work with participants in north east Victoria.


    Tasks involve assisting the client with one on one personal care support including tasks such as:

    • Transfers 
    • Personal Care – Full assistance with showering, toileting (including bladder and bowel care) daily skin checks, grooming and dressing
    • Rehabilitation Assistance –In home supported exercises program  in conjunction with physiotherapist
    • Meal preparation and assistance 
    • Domestic Assistance – All household tasks including sweeping mopping vacuuming, changing bed linen, cleaning bathroom and kitchen, clothes  washing, ironing, washing/drying dishes used and other tasks 
    • Documentation - Bowel/Bladder/Exercise charts, Communication book.
    • Shopping 
    • Community activites and events

    Minimum Requirements:

    • Current First Aid Certificate including CPR
    • Current Victorian Working with Children’s Check
    • Current NDIS Workers Screening Check
    • Certificate 3 in Individual Support, Disability, Community Services or equivalent industry related, or currently undertaking/willing to undertake.
    • Current Victorian Drives licence
