Plan Management enables you to have choice and control without having to do all the administrative paperwork. We support you and make sure you get the guidance and support to purchase the services which best meets your individualised plan and life choices.

Practice & Assessment

Our Practice and Assessment team are a group of practitioners who work together in supporting families to achieve their goals.

The team is made up of educators, behaviour specialists, speech pathologists, occupational therapists, allied health and advanced practitioners.

Support Coordination

ConneXtions Support Coordinators assist you to find individualised, quality supports in order to achieve your goals and to participate more fully in the community.

Your Support Coordinator will ensure that plan reviews commence three months before the end of your current plan and provide updates to the NDIS, monitor change in circumstances, complete progress reports and support you to fully engage in the National Disability Insurance Scheme to achieve the goals and outcomes identified in your plan.

Workforce Options

Are you looking for staff to implement your NDIS plan without the worry and hassle of finding the right person, training, supervision, and all the backend requirements?

The Workforce Team collaborate with your Support Coordinators to match staff to implement your NDIS goals and outcomes. All staff are trained and supervised to support you and your individual needs. To find our how to access our staff please contact us to discuss.

Support Options

ConneXtions Support Options team, based in Shepparton, provide personal supports to enable people living with a disability to complete day to day tasks and activities; at home, school, work and in the community.

Supports are available 24 hours a day, 7 days per week (including overnight support).
