What does a Support Coordinator do?

Our Support Coordinators assist you to find individualised, quality Supports in order to achieve your goals and to participate more fully in the community.

Six ways we provide support

Understanding how you can use your NDIS plan to meet your goals.

Your Support Coordinator works with you to develop a detailed program that aligns with the goals developed in your NDIS plan and how these goals will be achieved.

Initial assistance with linking you with the right providers based on your needs.

Your Support Coordinator provides assistance to source providers and coordinating a range of supports.

Managing Supports effectively to achieve the best from your plan.

Your Support Coordinator works collaboratively with you to personalise your supports and match you with the right staff and services.

Working with your network of support.

Understanding your network of family, friends, service providers, therapists, doctors and community resources. During the establishment phase you will meet with your Support Coordinator more often to set up your program of Supports. Once established and you're feeling more comfortable with your plan, you can work towards greater independence.

Accessing the NDIS portal and monitoring how your funds are spent.

As part of the process of working with you, we will support you to develop skills and confidence to take control of your NDIS plan. We can assist you to take over some of the Support Coordinator function yourself over time, and ask you for thing you need.

Monitor your progress and preparing for your next NDIA review.

Your Support Coordinator will meet with you regularly to monitor how your goals are being achieved and assist you to make any changes or adjustments you need. ConneXtions Support Coordinators will keep records about your progress so you have good information to use when you review your plan with the NDIA.

Register with ConneXtions

Your Support Coordinator will ensure that plan reviews commence three months before the end of your current plan and provide updates to the NDIS, monitor change in circumstances, complete progress and support you to fully engage in the National Disability Insurance Scheme to achieve the goals and outcomes identified in your plan.
