Practice & Assessment


Our Practice and Assessment team are a group of practitioners who work together in supporting families and participants to achieve their goals. The team is made up of educators, behaviour specialists, speech pathologists, occupational therapists, allied health and advanced practitioners.


Practice

Our Practice team take an innovative approach by working towards your goals through fun social opportunities. 

Assessment

We complete assessments to determine a starting point. In conjunction with families and participants, our team develop specific learning plans to meet your individual goals.

Social Learning Groups

We encourage social learning for children and adults by engaging in programs that develop new skills in an outcomes-focused social environment.

Step 1: Intake

After a family gets in contact with ConneXtions via phone, email or the Intake form, our intake team will get in touch and chat to you about your goals. We will explain how we work and the innovative approach we take to achieving those goals.

Step 2: Meet & Greet

Having decided to engage ConneXtions, families come in to meet our practitioners at Moorefield Park. During this first visit families receive a tour of our purpose built facility and discuss more specifically their goals. Throughout the discussion we learn more about your family's situation,  complete an initial assessment and make a follow up meeting for a formal assessment and observation. 

Step 3: Assessment & Observation

This is a two hour session where practitioners will talk with family members and interact with your child to get a better understanding of their development.


In this session, formal assessments are administered and may include:

  • Vineland-3 Adaptive Behaviour Scale
  • CELF-5 (Clinical Evaluation of Language Fundamentals)
  • Sensory Profile 2 
  • Social Responsiveness Scale
  • Functional Behaviour Assessment
  • SCERTS - Social Communication, Emotional Response and Transactional Supports


The participant is also invited to join in a social learning opportunity. This third observation provides the team with information regarding social interaction skills and behaviours.

Step 4: Evaluation of Assessments

The Practice & Assessment team meet and discuss formal assessments and observations. The Lead Practitioner will write a draft report in preparation to discuss with the family.

Step 5: Feedback to Families

The Assessment and Observation is discussed with families and this conversation determines the five specific goals that we work together to achieve. Goals are developed across the domains:

  • Communication
  • Daily Living Skills
  • Socialisation
  • Emotional Behavioural Regulation
  • Motor Skills.

Step 6: Plan Completed and Implemented

The Explicit Teaching and Learning Plan is presented to the family and we discuss how this can be implemented. Implementation through the Practice & Assessment team involves small social groups working on individual goals through circus arts, music, LEGO, minecraft and other engaging activities. These groups are planned and conducted by our practitioners and allied health support staff.
